Hallmark’s Professional Nursing Services include a variety of healthcare services. We deliver services according to your personal physician’s plan of care. Hallmark nurses and staff work directly with clinicians as needed to facilitate the highest levels of care possible. Our nurses work closely with our Home Health Aides, Physical Therapists, Speech Therapists, and Occupational Therapists when needed to coordinate a comprehensive care plan for your individual needs.
By working with your personal physician, nurse, and other healthcare workers, we are able to share information and ensure the appropriate level of care is provided. Updates on patient improvement and progression are shared with your provider to help him/her determine any additional needs or changes to a current plan of treatment.
