What is a Plan of Care?

Physicians often work with home health agencies to provide home care when your doctor believes it is safe and will provide you with the ability to improve your health. Hallmark Home Health is proud to serve Danville and the surrounding communities.

Patients have the right to choose the organization they want to use that provides home health services. Your choice should be honored by your doctor, hospital discharge planner, or other referring agency assuming your choice provides the required services and has availability. (Even though some hospitals have their own home health agency, you are not required to use their agency).

Hallmark Home Health works closely with your doctor to ensure his/her plan of care is followed closely.

All home health agencies are required to abide by privacy and confidentiality regulations. Your personal information is absolutely confidential.

Plan of Care

So, exactly what is a plan of care?

A plan of care is an outline of the services and care you should receive for your health issues. Your doctor works with your home health nurse and staff to decide on the services. They may include:

  • type of services needed
  • healthcare professional to deliver services (registered nurse, CNA, physical therapists, etc)
  • frequency of services
  • any needed equipment (durable medical equipment or DME)
  • dietary needs and counseling
  • expected outcomes from the plan (health improvement expectations)

Nurses review the plan of care with your doctor whenever necessary, but no less than every 60 days. Nurses have an opportunity to identify additional healthcare needs if they arise and will notify your doctor. They also keep your doctor updated on your health status and alert him/her if your health changes.

As long as you are eligible for home health services, we will continue to provide services according to your doctor’s plan of care. ONLY your doctor can make adjustments, additions, and other changes to your plan of care.
